Family Vaccine Record · Free
Every vaccine and date, kept on your device.
A free family vaccine record keeper. Record which vaccines each person has had and when, add travel shots and titer results, and print a tidy summary for the school nurse or the next checkup. Your records never leave your device.
A personal record you keep yourself. It is not an official immunization record and does not give medical advice.
The vaccine card is never where you need it
School forms, camp forms, a new pediatrician, a nursing program, a trip abroad: everyone asks which vaccines you have had and when. The answer is usually spread across a paper card in a drawer, two clinic portals and a photo on someone's phone. Titer results from a pre-employment blood test end up in yet another place.
Family Vaccine Record puts it all in one list per person, in a few minutes, without creating an account or handing your family's health information to another company.
How it works
1. Add your family
A first name or nickname is enough. A birth date is optional, only for your printouts. No account, no email.
2. Mark what each person had
Vaccines commonly required for school are listed first, then other common ones. Add the date, or mark "had it, date unknown". Add any other vaccine you want.
3. Keep titers and reminders
Record immunity blood tests with the lab's own interpretation, set your own reminder dates and add them to your phone calendar.
4. Print or back up
Print one person or the whole family, export a spreadsheet, or download a backup file you can restore on another device.
Private by design
- Only on your device. Records are saved in your browser's storage on the device you use. The tool makes no network requests with your data, and MedRise has no copy.
- Optional passcode with encryption. Set a passcode and the records are encrypted on your device (AES-256 with a key made from your passcode). The tool locks itself after 15 minutes without activity.
- No trackers. No advertising pixels, analytics or third-party scripts on the tool.
- You control the copies. Backups, spreadsheets and printouts are files you save yourself. Keep them somewhere private.
Because the records live only on your device, clearing your browser data or losing the device deletes them. Download a backup from time to time.
Which vaccines are required for school?
States, not the federal government, set school vaccine requirements, and they differ and change. According to KFF's August 2026 review, all 50 states and DC require MMR, DTaP, polio and varicella; 49 states and DC require hepatitis B; 35 states require meningococcal vaccine; and 27 states require hepatitis A. Exemption rules also vary by state.
The tool shows those vaccines first so they are easy to fill in, but it does not tell you what your child needs. Check your state's requirements and talk with your health care provider.
How to get an official record
A school or employer may ask for an official record. The CDC does not hold personal vaccination records; it suggests asking your health care provider, or your state's immunization information system (registry), which many states let you search. The CDC's records contact page lists each state. Once you have the official copy, Family Vaccine Record is a quick place to keep the dates handy.

FAQ
Where is my family's information stored?
Only in the web browser on the device you use. MedRise never receives it and the tool sends nothing over the internet. If you clear your browser data, the records are deleted, so download a backup from time to time.
Is this an official vaccination record?
No. It is a personal record that you enter yourself. Schools, employers and travel authorities usually need an official record from your health care provider or your state immunization registry. The CDC explains how to find them.
Which vaccines are required for school?
Each state sets its own list and it changes. According to KFF (August 2026), all 50 states and DC require MMR, DTaP, polio and varicella; 49 states and DC require hepatitis B; 35 states require meningococcal vaccine; and 27 states require hepatitis A. Check your state's current list.
Will it tell me which vaccines my child needs?
No. It records what each person has had and reminders that you choose. Decisions about vaccines belong with your health care provider.
Can I record titer (immunity) blood tests?
Yes. Enter the test, the date, the result exactly as written and the lab's own interpretation. The tool does not interpret results.
What if I forget my passcode?
The passcode encrypts the records on your device and is never sent to us, so it cannot be recovered. Keep a backup file somewhere safe.
